JOB SUMMARY

Are you an experienced Practice Nurse, Practice Manager or primary health care professional who is passionate about improving clinical practice, business capacity and patient outcomes?

Build strong productive relationships with Primary Health providers to help them improve clinical practice, business capacity and patient outcomes.

ABOUT SEMPHN

We are leaders, facilitators and influencers towards the shared goal of better primary health care.

Reporting to an independent Board, SEMPHN is funded by the Australian Government to help people in south east Melbourne get the health care they need, when and where they need it.

We do this by:

  • Commissioning out-of-hospital services, locally.
  • Partnering to make quality care more accessible and integrated, and easier to navigate – especially for people who need it most.
  • Helping primary health care professionals to deliver the best care possible – now, and into the future.
  • Influencing Government policy on primary health care reform.

Evidence-based practice is the foundation of our work. We are constantly asking, ‘together, how can we do this even better?’ This could mean challenging the status quo and designing things differently. Ultimately, it’s about measurably improving health for people in south east Melbourne. So, are you with us?

ABOUT THE ROLE YOU WILL:

  • Act as the SEMPHN’s front line staff; communicating PHN priorities to the primary health care workforce,
  • Build strong relationships with General Practices and other providers of primary healthcare to support the facilitation of change,
  • Actively help build the capacity, skills, and knowledge of primary health care providers,
  • Plan, coordinate, implement and support a variety of projects which focus on coordinated care and chronic disease management,
  • Support the development and implementation of commissioned activities.

This is a maximum term position for 24 months.

You will be based in Heatherton, with travel across the SEMPHN catchment.

WE WOULD LOVE TO HEAR FROM YOU IF YOU HAVE:

  • Tertiary qualifications in either nursing, health, social sciences and/or extensive relevant experience.
  • A good knowledge of contemporary issues for general practices and the primary health sector.
  • Proven ability to influence positive change to general practices and primary health care providers at the micro level.
  • Demonstrated relationship management experience.
  • Experience in delivery & coordination of projects.
  • Project management skills in planning, implementation and monitoring, and report writing skills.
  • Presentation skills, ability to multi-task, intermediate to advanced skills in Microsoft Office.
