Our Youth Partnership Project (YPP) is an innovative and collaborative early intervention strategy, getting the right support, to the right young people, at the right time. We're looking for an experienced, driven individual to join our team and create meaningful change for young people in Perth's south-east corridor. In this leadership role, you'll coordinate an integrated case management approach that supports young people and families in a holistic and responsive manner. No two days will look the same; your responsiveness, problem solving, and flexibility will be drawn upon daily to ensure young people are able to access the right supports to make positive decisions.

Where you come in:

In this full-time, fixed term position (through to August 2021, with potential for extension), based across Armadale and Gosnells, you'll work to provide expert guidance to the broader team to best support families of the young people identified by the project. Reporting to the Team Leader, your work will ensure quality and cohesive service coordination that holistically supports a reduction in risk factors, providing young people and families with opportunities to develop and thrive.

You will make an impact by:

  • Building strong relationships between young people, their families and the service system
  • Facilitating regular face to face meetings and ongoing communication between the YPP's cross-sector of stakeholders, organisations and clients
  • Strengthening and building upon current family support approaches, implementing evidence based and best practice recommendations
  • Developing and maintaining a high standard of progress reporting, evaluation and data collection

You're truly passionate about best practice in early intervention for young people and their families. You understand the stresses on young people with complex needs and the range of risk and protective factors which impact on life outcomes. Your cultural knowledge and stakeholder networks will help you build strong connections with people from a variety of cultural and socio-economic backgrounds. Ideally, you have a strong understanding of community development principles and practices, with a commitment to achieving outcomes through building the collaborative capacity of stakeholders.

This role requires:

  • Tertiary qualification in Human Services, Social Science or another relevant field
  • Significant experience working within a case management role and/or in a collaborative partnership setting
  • Demonstrated experience establishing and maintaining positive engagement with families and young people with complex needs
  • Demonstrated ability to effectively establish and maintain strong working relationships with a variety of key stakeholders, including government agencies
  • Experience working with Aboriginal young people and their families
  • Driver's licence (car available for work use)
