Every day of the year, Sacred Heart Mission assist hundreds of people who are experiencing homelessness or living in poverty to find shelter, food, care and support. We are in this for the long haul - we are committed to making a sustainable difference in people’s lives. Everyone is welcome at our table.
The GreenLight Supportive Housing Program is run in partnership with VincentCare, and Salvation Army. We support people who have experienced chronic homelessness including rough sleeping to establish a home and community connection, provide specific interventions to sustain tenancies, and will support underlying health and other issues that may impact on people staying housed.
As one of a small team of Supportive Housing, Case Managers, you will spend your days working with other team members and clients to support, to settle into housing, stay housed, and improve their quality of life. The role will see you working in the community and in people’s homes as you assist them to develop goals and navigate services that meet their needs to stay housed. You will be based in the heart of St Kilda supporting clients across the South and inner metro regions of Melbourne.
A typical day will see you working with other team members and clients, delivering case management and service coordination activities, and liaising with other agencies to support people to end homelessness.
