The GreenLight Supportive Housing Program is run in partnership with VincentCare, and Salvation Army. We support people who have experienced chronic homelessness including rough sleeping to establish a home and community connection, provide specific interventions to sustain tenancies, and will support underlying health and other issues that may impact on people staying housed.

Your new role

We are looking for someone who is interested in stepping up as part of their career progression. This is an opportunity to work with 3 different organisations. The role includes Client support as well as training staff and secondary consults. Experience in working Mental Health or AOD sector desirable. We have 2 roles available Full time and part time.

The role is one of two Health and Wellbeing Worker/s roles in the GreenLight program. The role will be located on GreenLight teams based across the inner Melbourne area by SHM, VincentCare, and Salvation Army and will use assertive engagement and outreach as the primary service delivery mode.

Your day to day

  • Coordinate, facilitate and integrate health and wellbeing support, care and support
  • Provide brief intervention, safety assessment and crisis management as required.
  • Provide case management and support coordination to clients in collaboration with GreenLight Case Managers.
  • Develop and foster partnership with key mental health/AOD/NDIS/health service providers.
  • Liaise with and review care coordination plans with clients, families/ carers to support recovery.

The person we're looking for

The successful candidate will have:

  • Practiced and skilled health and wellbeing workers
  • Experience working within mental health/AOD/general health/disability (including NDIS)
  • Commitment to recovery and person-centred principles
  • Experience working with people with complex needs and/or homelessness and housing
  • Experience in multi-disciplinary teams and inter-agency partnerships
  • Resilience, flexibility and adaptability
