Your new role:

Our Sacred Heart Mission Local (Local) program supports people in the community through the Commonwealth Home Support Program (CHSP), Home Care Packages (HCP) and National Disability Insurance Scheme (NDIS) program.

Your day-to-day:

A typical day will involve:

  • Liaising with clients to give and receive updates
  • Coordinating rosters
  • Establishing care and supports
  • Scheduling
  • Updating records
  • Liaising with support staff and care managers
  • Ensuring best practice through timely and accurate information.

You’ll facilitate daily living for our clients which is the key to remaining safe in their home and to maintain their relative independence.

You’ll be a dynamic part of the ongoing development of services at Local as we strive to provide the best possible care to people in the community with complexities, such as homelessness, marginalisation and social isolation. Working as part of the CLO team, your strong alignment to the values of Sacred Heart Mission will drive you to make a real difference to the lives of those we support.

We're looking for

  • Compassionate people to join our team on a full-time basis
  • Experience in a CLO or rostering role
  • Working knowledge of rostering software such as TCM, SRS or Humanforce
  • A driver’s licence
  • A current first aid certificate including CPR
  • Experience with multiple and complex care needs (desirable)
  • Motivation to help people maintain their independence
