RSB supports people who are blind or have vision impairment to gain new skills, be engaged in life and grow confidence at home, at work and in their communities. The RSB is looking to appoint two Aged Care Support Facilitators on 12-month contracts to initiate outbound contact with customers eligible for Commonwealth Home Support Programme (CHSP) funded services on a routine basis for service reviews or specific projects.

The successful applicant will:

• Support existing customers eligible for CHSP funded services to engage with RSB services;
• Produce timely and accurate customer and service documentation to meet RSB and aged care funding requirements, as documented in RSB policies and procedures;
• Engage with customers and customer representatives in a positive, customer centred, supportive manner.

The successful applicant will have:

• Excellent interpersonal and communication skills, including the ability to relate to persons of all ages, including those with a disability;
• Excellent organisational skills with an ability to prioritise and complete work within established deadlines;
• Ability to recognise and manage customer needs within the context of organisational responsibilities, service capability and funding obligations;
• Disability Certificate IV, Aged Care Certificate IV or similar qualification that includes care coordination or case management.

A sound knowledge and understanding of vision impairment and empathy for people who are blind or vision impaired is highly desirable but not essential.

A National Police Clearance and satisfactory Department of Human Services Disability and Aged sector screening and Working with Children clearances are required.
