Be part of an energetic team providing transitional support to young people in outer Gippsland.

THE ROLE 

• Build rapport with young people whilst in care and contribute to transition planning.
• Provide case management support to young people leaving care with a focus on long-term outcomes.
• Engage in assertive outreach and support that includes varying levels of intensity.
• Motivate, encourage and empower clients to address areas of personal difficulty and consider options for the development of personal wellbeing.
• Actively promote the establishment and maintenance of positive family relationships.

THE IDEAL CANDIDATE – what you will bring;

• Working knowledge of the child protection system and the complexities of young people transitioning from care.
• Demonstrated understanding of the impacts of trauma and the social and political issues impacting young people
• Ability to engage, build relationships and network with the community to support young people transitioning from care. 
• Valid Victorian Driver’s Licence |Working With Children's Check |Police Check
• Minimum requirement of Diploma of Community Services or other relevant or appropriate tertiary qualifications and/or experience relevant to this position.
