ABOUT THE ROLE

We are looking for self-motivated candidates to apply for the Case Manager position as either a Peer Treatment Facilitator OR a Treatment Support Facilitator.

Peer Treatment Facilitators support the delivery of treatment support services to People Living with HIV (PLHIV) to improve engagement in care and treatment and reduce stigma and discrimination towards the PLHIV community.

The primary purpose of the Peer Treatment Facilitator (PTF) is to provide client-centred peer navigation and/or case management support to PLHIV who are experiencing barriers, which negatively affect their ability to commence and/or maintain HIV treatment and care. This role requires the use of the lived experience of HIV and the application of case management and self-management framework, as such is required to be filled by a person living with HIV who is comfortable sharing their experience with clients and the community. The PTF will assess client circumstances and needs holistically and provide peer education and practical assistance to navigate the health system. The PTF will also build tailored action plans in collaboration with clients to overcome identified barriers and address the social determinants of health that impact treatment uptake, adherence, and retention in care.

Treatment Support Facilitators support the delivery of treatment support services to People Living with HIV (PLHIV) to improve engagement in care and treatment and reduce stigma and discrimination towards the PLHIV community.

The treatment support case management role, with a strong focus on client empowerment, is based in Cairns and provides a variety of experiences and learning opportunities for staff. The role requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment as well as comprehensive project management, time management, facilitation and event coordination skills. The TSF will also build tailored action plans in collaboration with clients to overcome identified barriers and address the social determinants of health that impact treatment uptake, adherence, and retention in care.

We’re interested in candidates with extensive knowledge and experience of the HIV sector and the passion and drive to bring new ideas to the organisation that will support a diverse range of our clients to achieve optimal health and emotional wellbeing.

Working in a small team of dedicated, dynamic staff and volunteers committed to social change and action, you will have excellent program coordination, time management and communication skills, and strong attention to detail. You will be client focused, an excellent communicator and exhibit strong empathy, compassion and cultural awareness.

This SCHCADS Award level 5 position is offered full time, and located in Beenleigh with outreach covering the region Gold Coast and west to Toowoomba. Generous salary packaging is available.
