On behalf of our customer, we are seeking a Case Manager to provide case planning support for young people who have experienced traumatic experiences in their life including alcohol abuse, drug abuse, homelessness and domestic violence.
Key Responsibilities:
- Develop case planning in collaboration with the young person and residential worker to determine risk and needs assessments
- Support and assist residents/young people to secure appropriate educational or training pathways and provide both on-site and outreach/mobile responses; including visits to the home, schools and other education facilities
- Build strong relationships with mainstream education and employment services for the benefit of young people
- Support the Team Leader in developing and progressing the education and training program which is part of the refuge
- Record and assess referrals received from various agencies on the behalf of young people requiring the services of the Refuge
- Participate in the on-call roster
- Provide continued services to those clients that have exited from the intensive support team accommodation program as required
- Be involved in the action which positively influences the policies and social factors that affect at-risk young people
- Ensures client records are maintained in a confidential manner
To Be Successful:
- Formal qualifications ( Min. Diploma in community service or related field)
- Demonstrated extensive case management experience in child protection, homelessness, youth support or similar essential
- A demonstrated understanding of the needs of young people who are homeless or at risk of homelessness
- Demonstrated experience working with youth at risk
- Ability to conduct a thorough assessment and intake processes and make appropriate recommendations and referrals
- Capacity to work with minimal supervision but able to work as part of a team
- Current Working With Children's Check or the ability to obtain
- Current police check or ability to obtain
- Current NSW Driver License
