The purpose of the role of NDIS Support Coordinator is to provide support coordination to NDIS participants in interpreting their NDIS plan, understanding their funding, assisting them to research and implement NDIS funded supports and to build their capacity over time enabling the participant to coordinate and manage their own supports and funding.

OBJECTIVES:

  • Operate in accordance with NDIS legislation and Recovery Plus Support’s policies and procedures and values.
  • Maintain confidentiality of participants, their families/carers and the organisation
  • Network internally and externally to support Recovery Plus Support’s goals
  • Provide NDIS participant focused progress reports within the agreed reporting frequency
  • Effectively utilising systems provided by Recovery Plus Support

KEY RESPONSIBILITIES:

  • Working with NDIS participants to achieve their goals in line with their NDIS plan.
  • Record accurate case notes in relation to the participant’s progress on Recovery Plus Support’s CMS
  • Ensure Work Health and Safety guidelines are adhered to.
  • Complying with relevant legislation and codes of conduct.
  • Ensure knowledge of NDIS systems and practices is current and up to date
  • Participate in all training and workshops provided by Recovery Plus Support
  • Ensure professional boundaries with participants are maintained
  • Maintain and update Recovery Plus Support’s Support Coordination Resource and Succession Plan
  • Assist NDIS participants to implement their plan and build their capacity to manage and direct their NDIS funded supports
  • Arrange implementation meeting with NDIS participant upon accepting referral (Meeting can take place at their home, office or their designated location of choice)
  • Assist NDIS participants to increase their informal and formal networks
  • Build capacity to understand and ensure NDIS funded supports are value for money
  • Access and use the NDIS Portal (Proda)
  • Fund Utilisation
  • Provide support to research and apply for SIL/ SDA/ ILO
  • Effectively communicate and maintain relationships with the NDIS and other stakeholders
  • Research and refer informal and formal support / allied health providers
  • Provide sufficient information to enable the participant to select their preferred provider options
  • Support to develop, understand and sign service agreements
  • Arrange assessments to determine SDA/ SIL/ ILO eligibility, complete application process, complete Housing Plan
  • Arrange allied health referrals and assessments
  • Determine the breakdown of available funds for each NDIS funded support
  • Liaise with support providers and plan manager to establish the appropriate line items as per NDIS Pricing Arrangement
  • Build the capacity of NDIS participants to coordinate supports, self-direct and manage their supports.
  • Support the participant to engage with providers, including providing assistance with: resolving problems or issues that arise, understanding their responsibilities under service agreements and changing or ending a service agreement
  • Support participants to understand and execute service issues, complaints, major changes and plan reviews.
  • Provide NDIS participant focused progress reports within the agreed reporting frequency
  • Assist participants to prepare for their plan review by supporting them to: obtaining progress reports and quotes from NDIS funded support providers, review current goals, identify new goals and barriers to implementation
  • Build ability to exercise choice and control
  • Assess and manage risk in line with organisational policies and procedures
  • Assist with Review of a Reviewable Decision process
  • Assist with Change of Circumstances process
  • Maintain regular contact / check in’s with participant or plan nominee
  • Monitor NDIS Support Coordination funding to ensure sufficient funds remain throughout the duration of the current NDIS plan
