Pride Disability Services (Pride Vic) is a privately owned Disability Support Service based in Newport, Victoria. The staff at Pride Vic focus on delivering person centred, quality and dedicated supports and services to LGBTQIA and gender diverse individuals, who have intellectual, physical, or mental health disabilities.

We provide a non-threatening, non-judgmental and safe environment where individuals can access gay, lesbian and other community groups.
We bridge the gap for the people of the LGBTQIA and gender diverse community, who have intellectual, physical, mental health disabilities, their caregivers, families and friends, and mainstream society.

Pride Disability Services (Pride Vic) is seeking an experienced Support Coordinator to assist individuals to understand, implement and utilise their NDIS funded plan. The Support Coordinator will provide choice and control to the participant while connecting them with necessary services and providers to build their capacity for independence and engagement in the community.

Key Responsibilities:

  • Provide assistance to strengthen participant’s ability to understand and work within the NDIS framework.
  • Manage an assigned case load to support the commencement and implementation of funded supports in the participant’s NDIS Plan, incorporating informal, community and mainstream supports.
  • Complete Service Agreements with Participants / informal supports and funded supports, reach decisions regarding services they choose, resolve service delivery issues, and address barriers to their participation.
  • Monitor and review spending and quality of services and programs in the participant’s NDIS Plan, including tracking of billable hours.
  • Ensure support needs and risk assessments are conducted for participants with complex support needs, interpret complex information about functional limitations, and follow up with service providers to ensure participant’s needs are met.
  • Participate in the formal review of the participant’s NDIS Plan when requested by the participant and/or as specified by the NDIA Planner.
  • Liaise and work professionally with NDIA Planners and Local Area Coordinators to gather and verify information to facilitate plan implementation.
  • Ensure high quality record keeping and provide NDIA with reports on outcomes and success indicators within the agreed reporting frequency.
  • Ensure that the National Disability Insurance Scheme Act 2013 legislation is maintained in accordance with the role.

Skills and Experience

  • Diploma in relevant Social & Community Services or equivalent experience
  • Demonstrated knowledge of the NDIS and Quality & Safeguards Commission
  • Knowledge of NDIS Act 2013, Disability Act 2006 and other relevant legislation
  • Prior case management experience

To be Successful you must have:

  • Excellent verbal and written communication skills, as well as intermediate knowledge with Microsoft Office programs
  • Excellent Networking Skills – collaborating with multiple stakeholders including informal, community and mainstream supports
  • Excellent time management and administrative skills
  • Strong case management and engagement skills
  • The ability to think innovatively and creatively to help clients achieve their NDIS plan goals
  • The ability to work autonomously and cooperatively as a member of a team
  • The ability to maintain & provide service user and staff confidentiality
  • Capacity to engage constructively and respectfully with individuals of diverse backgrounds and abilities to achieve high quality outcomes
  • Current full Driver’s Licence and own vehicle
  • Current certificate in First Aid & CPR
  • NDIS Worker Screening Check
  • Working With Children Check (Employee)

Offering:

Flexible employment at an hourly rate, permanent part time to be negotiated with the successful applicant with a 6 month probation period at our head office in Newport, Victoria.

Pay rate as per (SCHADS) award Social, Community, Home Care and Disability Services Industry Award [MA000100]
