What You Will Be Doing

We are looking for a passionate community Occupational Therapist on a full time/part time, permanent basis, based at our Community Health sites. Join our team and positively impact the lives of our local community!

The Occupational Therapist assists eligible clients (HACC/CHSP) by providing comprehensive home assessment; specialist recommendations regarding assistive equipment and home modifications; holistic client/carer support and education; episodic care co-ordination and referral. You will be working with clients from various socio-economic and cultural backgrounds in a variety of settings including the home, supported accommodation, and the community. Interpreter services are readily available to promote client-centred practice. These positions are based at our Mornington Peninsula Community Health sites.

We partner with the local City Councils, Monash University Peninsula Campus, and external industry partners to develop innovative projects. Examples include the Rapid Response Program for HACC initial assessments conducted with Council Home Maintenance teams to shorten the program waitlist dramatically, as well as lecturing and support of local university students. Past Honours student placement projects have led to quality initiatives and development of network wide resources. Current projects include refining our initial assessment documentation including care plans, use of outcome measures, and specific assistive technology tools.

What You Need

We anticipate you will have:

  • Do you hold a Tertiary qualification in Occupational Therapy and current APHRA registration?
  • Are you able to demonstrate current specialist knowledge and skills in the provision of evidence based occupational therapy services in a community setting?
  • Do you have skills in home assessment and intervention, including complex equipment and home modification prescription?
  • Do you have current registration as a prescriber with the State Wide Equipment Program (preferably with some Amber and Red credentials)?
  • Do you love working with clients on their day to day and long term goals?
  • Do you love being involved in new and innovative ideas to assist with independent living?
  • Are you able to listen, without judgement, to gain understanding of a persons current and previous lived or living experience?
  • Do you thrive within a team environment of highly driven heart centred people?
