We currently operate in a number of NDIS regions, and looking to expand our team with a role currently available for to commence in January 2021 in the Western Melbourne Region (Sunshine) as a team leader.

About the role

To ensure the team you lead provide high quality Support Coordination for NDIS participants who have disabilities, and facilitate the implementation of all supports identified in the participant's plan. The role is focused on strengthening the Support Coordinators ability to navigate the NDIS and empower participants to access and coordinate supports to enable each person to participant more fully in their community of choice. Through this strengthening of Support Coordinator capability, you will improve team productivity to meet and exceed targets, improve team knowledge, and promote self sufficiency and problem solving within the team.

Key Responsibilities:

  • Ongoing review and monitoring of changing NDIS legislation and operational requirements, including changes in NDIS price guide and/or policy
  • Ongoing review of team performance and utilisation against assigned KPI's ensuring performance development planning and review is conducted for all staff in accordance with Pathways to Care's policy
  • Respond to points of crisis within the team's case-load when required
  • Work in close consultation with the Intake Coordinator to review the intake list, ensuring suitability and the timely allocation of referrals
  • Provide regular reports and updates to management on both strategic and operational matters, development of the team and participant issues
  • Maintain a case load and deliver NDIS Support Coordination within the NDIS Funding Frameworks, Terms of Business and Operational Guidelines against assigned KPI's
  • Manage all risk management issues for the team, in close consultation with the Senior Management Team, following current risk management policies
  • Provide role specific job training of Support Coordination staff
  • Provide clinical supervision to Support Coordination staff to ensure safe delivery of services

Skills and experience

  • Experience as an NDIS Support Coordinator
  • A tertiary qualification in Social Work, or other social, or health sciences is desireable
  • Demonstrated experience - minimum 2 years - leading a small team to achieve targets
  • Previous experience in achieving billable hours targets, monitoring your hours of billable time and growing the teams productivity
  • A self-starter, self-motivated person who can work independently
  • Demonstrated experience working with people with a disability; mental health work and a sound understanding of psychosocial recovery within a human services context
  • Extensive knowledge of local disability, mainstream and community services
  • Coaching and mentoring skills to develop and support the team of Support Coordinators.
  • Excellent verbal communication skills and a demonstrated ability to engage with staff, participants and their families
  • Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence and other documents
  • Prefer someone experienced with cloud-based Customer Relationship Management system
  • Valid Victorian Driver's Licence, reliable car and comprehensive insurance
