Pathways to Care currently operate in Loddon, Inner and Outer Eastern Melbourne, Western Melbourne and Goulburn NDIS regions, and are seeking a community services professional (Social Work)/allied health worker to join us as an Clinical Lead/Specialist Support Coordinator in our Loddon/Goulburn team. The role is full time and based in Bendigo. Flexible Working Arrangements are on offer for the right person.

About the role

Reporting to the National Manager of Support Coordination Services, the primary objective role is to provide high quality Specialist Support Coordination for NDIS participants and Clinical Guidance for members of the Pathways to Care Loddon/Goulburn regions.

The Clinical Lead/Specialist Support Coordinator is primarily responsible for providing clinical leadership and direction to the Support Coordination team, ensuring that day-to-day clinical work is carried out to effectively to meet the needs of Pathways to Care Participants.

In their direct client work, the Clinical Lead/Specialist Support Coordinator will focus on strengthening a participant's ability to navigate intersecting service systems, including Health, Justice, Mental Health, Housing, and other Social Services. The incumbent will contribute to the development of evidence-based and innovative clinical services and that will promote the delivery of Specialist Support Coordination of the highest quality.

Key Responsibilities:

  • Provide Level 3 Specialist Support Coordination to a high standard, as expected for a specialist position.
  • Provides required reporting based on the Specialist Support Coordination Framework, as directed by the NDIA.
  • Applies an advanced knowledge of the Australia Social Services System, and the channels to access relevant supports within each.
  • Ensure mainstream services meet the obligations to the participant (Housing, education, justice, health), displaying confidence and competence in drawing together representatives of each service system in networks of support, as required for complex care team meetings
  • Support the participant to implement their NDIS plan; monitor plan outcomes and expenditure to ensure the plan remains in budget
  • Build effective relationships with participants and their families while maintaining appropriate and professional boundaries, including the limits of own job role.
  • Clear and accurate record keeping of information, time and billing
  • Primary source of secondary consultation around areas of clinical complexity or risk emerging from the Support Coordination team in the Loddon/Goulburn regions

  • Salary range - $76,412 Inc Super - $84,454 Inc Super FTE - dependent on relevant experience

Skills and experience

Prior Support Coordination or demonstrable experience in Community Sector Case Management will be highly advantageous

  • Previous experience in achieving billable hours targets, monitoring your hours of billable time
  • A self-starter, self-motivated person who can work independently
  • Tertiary qualification (or near completion) in either Social Work, Psychology or other Allied Health
  • Demonstrated knowledge of psychosocial diagnoses, both chronic and acute, or the ability to source accurate and critical patient information.
  • Current knowledge of restrictive practice guidelines, and awareness of mandatory reporting frameworks (Child Safe and NDIS Commission)
  • Solid background and knowledge of NDIS
  • Extensive knowledge of local disability, mainstream and community services
  • Excellent verbal communication skills and a demonstrated ability to engage with participants and their families
  • Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence and other documents
  • Prefer someone experienced with cloud-based Customer Relationship Management system
  • Valid Victorian Driver's Licence
  • Reliable vehicle
  • Must be able to successfully pass probity checks: National Police Check, International Police Check if relevant, Working with Children Check, Disability Workers Exclusion Scheme (DWES)
