A support co-ordinator plays a critical role in supporting participants (and families) to understand their NDIS plans, navigate the NDIS system and the interfaces into the broader system of services and to build their capacity to direct their supports independently over time.

You will work directly with NDIS participants to develop plans to support them to live a fulfilling and meaningful life at home, in the community and achieve their goals.

Using an individualised approach, you will explore/investigate and coordinate a variety of services for participants, by facilitating effective working relationships and communications with the carers, family, external services, and other key stakeholders to help provide the most well-rounded support available.

This is a Part time role based in our Yarrawonga Office and reports directly to the CEO.

SKILLS AND EXPERIENCE

  • Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice)
  • NDIS awareness, particularly in relation to support coordination and the importance of choice and control for participants.
  • Demonstrated ability to develop networks and positive relationships with a range of participants, their families, and their service providers.
  • Well-developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language to deadlines.
  • Strong interpersonal skills with an understanding of diversity when working with participants and colleagues alongside great negotiation and conflict resolutions skills.
