About the role

An exciting opportunity is available for the right person to join our NDIS Support Coordination Team in the role of Specialist Support Coordinator. You will have considerable experience working with participants with complex needs, mental illness and/or complex psychosocial disability, and hold a Psychology, Social Work, Occupational Therapy or other Allied Health disciplines (essential)

Our role is very much focused on driving clear participant outcomes. Striving to ensure that quality services and frameworks are put in place for our participants with complex needs that will assist them in achieving goals as set out in their NDIS plans. Your strong stakeholder management skills and ability to help navigate through a process will be critical to your success in this role.

Key responsibilities will include:

  • Providing Specialist Support Coordination in line with ONCALL’s Specialist Support Coordination Framework and in accordance with NDIS Practice Standards
  • Providing person centred services that enrich the lives of people living with disabilities
  • The development of complex service plans to address barriers
  • Negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation
  • Ensure that all funding management activities are undertaken, and that participants are able to maximise outcomes outlined in individual plans
  • Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders as required
  • Ensure that all participants client are up to date, complete and are of a high standard
  • Ensure all mandatory reporting is undertaken and required assessments completed within timelines and of a high quality

To be considered for this opportunity you will have:

  • Relevant tertiary qualification (in Psychology, Social Work, Occupational Therapy or other Allied Health disciplines (essential)
  • Demonstrated experience a disability setting, including working with participants with complex needs and barriers, including mental health and complex psychosocial disability
  • Demonstrated experience and expertise in planning, assessment, referrals and case management implementation
  • A thorough understanding of the NDIS, Practice Standards and relevant sector reforms
  • High level written and verbal communication skills
  • Excellent time management and organisational skills
  • A current (or willing to obtain) police check, WWC check, NDIS Workers Screening check and DWES check
