Your Skills and Experience

Reporting to the Manager Out of Home Care, the Senior Case Manager is responsible for the effective case management, delivery and reporting of Target Care Packages and In Home Support, including leading a small team and engaging in assessment planning, facilitation, care coordination, advocacy and support to children, young people and families. The goal is to meet individual and family health needs, achieve cost-effective outcomes, identifying and submitting written TCP proposals to DFFH.

This position will also hold a small case load and strive to provide high quality emergency, transitional, residential and in-home support to our CYF participants.

To achieve this, you will possess:

  • Significant experience in case management working with children, young people and families
  • Extensive experience in Out of Home Care, Family Services and Child Protection including planning and implementing and reviewing all aspects of provision and delivery of OOHC services and relevant legislation
  • Experience in managing a team, and providing supervision/reflective practice
  • Experience in preparing reports and evidence to the DFFH and the Courts as requested for children subject to Children's Court
  • Demonstrated ability to build and maintain collaborative and professional working relationships with clients and internal/external stakeholders
  • Demonstrated ability to conduct risk assessments and problem solve issues affecting high risk CYF clients
  • Ability to liaise and build rapport with families, direct care workers and other professionals
  • Effective and engaging communicator with proven ability to mentor and inspire direct care workers in OOHC
  • Meet the minimum Recognised Qualification criteria, as outlined in the Minimum Qualification Requirements for Residential Care Workers (Vic)
