About the opportunity

The RAS Assessment Officer has an integral role in assessing the needs of community members for eligibility under the Community Home Support Program and Home & Community Care Program for Younger People. Work with clients to assess their needs and develop an appropriate support plan that focuses on the strengths of the clients that continues to build independence and fosters the client's ability to remain living at home with appropriate supports.

The role is based across all sites including Wallan, Broadford, Seymour and Kinglake as well as providing Outreach services.

To be successful in this role, you will have:

  • A tertiary qualification in Allied Health, Social Work, Disability, Nursing or Community Services.
  • First Aid and CPR Certificate.
  • Experience and knowledge of community based services and an understanding of issues facing the aged and people with a disability is preferred but not essential.
  • A general knowledge and understanding of assessment and care planning, using person-centred care models.
  • Well-developed time management, IT and administrative skills and are able to competently use service portals and databases and can work in an environment that has strict timeframes.
  • Received both doses of the COVID-19 Vaccine.
