Nexus Primary Health supports people to fully participate within their community through the provision of appropriate wide-ranging support services throughout their life span. With a 40-year history, we are the "go-to" organisation for primary health information and person-centred services in the outer and rural north of Melbourne.
Health Systems Navigators - Administration establish, identify and improve client health outcomes. They focus their administrative duties towards care coordination based on client needs, assisting with the management of appointments, wait-list triage, collation of pathology results, appointment documentation and follow up of clients as per their health plans.
The role will also assist in the operation of ancillary clinics, assisting with client bookings, processing of referrals and attending to the general functions of the clinics.
As required, staff will rotate through all Nexus locations (Wallan, Broadford, Seymour and occasionally Kinglake) within rostered opening hours and may be asked to back up their colleagues to cover leave.
You will need a driver's licence and vehicle and be able to travel between Nexus sites when required. You will also need to be available to work across a 7-day roster, between the hours of 8 am to 8.30 pm. We are looking for a minimum of 20 - 24 hours availability per week. Penalty rates apply for after-hours and weekend work.
