About the Role

  • Full Time, Maximum Term Contract until 30 June 2021
  • CSD Level 2 from $63,670 - $68,264 per annum (pro rata) + super

The NDIS transition worker role will be based at the Neami Hurstville site to deliver the National Psychosocial Support Measure – Extended Transitional Arrangements for Commonwealth Community Mental Health Clients (NPSM2). This program aims to support consumers who have been previously engaged in programs such as PHAMS and or PIR, and who are yet to complete the NDIS application process. The primary focus of this service will to test NDIS eligibility for all consumers in the service who have not yet done so, and consequently, transition consumers to appropriate support provisions, either with the NDIS or Continuity of Support services: both being person centred and aimed at achieving a better quality of life as identified by the consumer.

Core duties

  • Engage consumers and develop trusting and professional relationships
  • Support consumers to test for NDIS eligibility
  • Maintain records and resources
  • Work with consumers to gather evidence to demonstrate psychosocial functional impairment
  • Ensure transition to appropriate NDIS services

What will you bring to the role?

  • Relevant experience in a similar role
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives.
  • Knowledge of complex mental health and drug and alcohol issues.
  • Knowledge of the NDIS application process
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

Mandatory requirements

  • Current NSW Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
