About the program:

Continuity of Support (CoS) provides non-clinical psychosocial support to consumers who previously accessed the Partners in Recovery (PIR), Support for Day-to-Day Living in the Community (D2DL) and Personal Helpers and Mentors (PHaMs) programs. The service supports people with complex mental illness who have been assessed as ineligible for the NDIS.

Services delivered under CoS are predominantly group-based and focused on building individual capacity, with additional targeted one-on-one support at times of increased need.

CoS is delivered by Wellness Connect, a consortium of mental health and community organisations led by Neami National and comprising Mission Australia, UnitingCare Wesley Bowden, Life Without Barriers and Community Access and Services SA. We work together towards a common purpose: to improve the health and wellbeing of people experiencing mental ill-health.

Wellness Connect are recruiting for two roles to be situated within Neami National and Life Without Barriers.

About the role:

As an NDIS Access Officer-CoS within Wellness Connect, your role will be to review Continuity of Support Consumers for potential eligibility to the NDIS and assist those selected to test their eligibility with the NDIA. You will also contribute to the transition of consumers to NDIS providers where eligibility and access is granted.

  • Work with CoS support workers in identifying existing CoS consumers suitable for retesting for the NDIS
  • Connecting with and assisting people previously assessed as ineligible for the NDIS to retest their eligibility for the NDIS and facilitate entry into the scheme
  • Assist those determined eligible for the NDIS to transition to NDIS supports, working closely with the consumer’s allocated CoS support worker where that exists, if access is approved and whilst they are awaiting their NDIS plan and supports to be put in place
  • Full Time/Part Time, Maximum term contract
  • Office hours Monday to Friday 8:30am to 5:00pm
  • Based in Neami National, Wright Street or Life Without Barriers

The scheme is established to provide support for people who have a permanent or likely to be permanent disability and substantially reduced functional capacity. Its intent is to give individuals:

  • Holistic person centred planning
  • Tailored individual funding packages and portability

Mandatory Requirements:

  • Criminal record check required before commencement of employment
  • Working with Children and Vulnerable Person check required
  • Minimum Certificate IV or tertiary qualifications in human services or a related discipline
  • Previous consumer service experience within mental health or demonstration of consumer service attributes
