Towards Home+ as part of the Victorian Homelessness and Rough Sleeping Action Plan (2018), has the following suite of services:
As a Community Rehabilitation and Support Worker, you will provide assertive outreach support to people who are homeless and sleeping rough. This includes intensive, flexible, responsive, client focussed support with the aim of assisting clients to successfully sustain housing, improve physical and mental health and develop connections within the local community.
Neami National is a community mental health service supporting people living with mental illness to improve their health, live independently and pursue a life based on their own strengths, values and goals.
