About the Role

  • Part Time, 0.8 FTE
  • 64k – 69k p.a (pro rata) + super + salary packaging + benefits
  • Maximum Term Contract until 1 March 2023
  • Based in Broadmeadows

Neami National has won two years’ funding for a new Victorian Government initiative – Homelessness to a Home (H2H). As part of the Initiative, the government is investing to provide housing and support to those currently experiencing homelessness and who have been accommodated in hotels during the Covid crisis. In Hume/Moreland/NE Melbourne, the service will be delivered in full partnership with Homes Victoria, Housing Choices Australia (HCA), Bolton Clarke, Uniting ReGen, and Merri Community Health.

The service will incorporate assertive outreach and engagement, intensive case management and housing support to dwellings managed by HCA or Homes Victoria.

Neami’s role is to deliver high intensity, flexible care to these consumers for the duration of the program including providing support with:

  • maintaining their tenancy through improved daily living skills
  • gaining access to clinical health and mental health services
  • engaging with local community

This position is part of the H2H Team, a joint support initiative led by Neami and partnered with Housing Choices Australia, Uniting ReGen, Bolton Clarke and Merri Community Health.

Core duties

  • Engage consumers and develop trusting and professional relationships
  • Intensive case management support to people sleeping rough
  • Tenancy establishment and maintenance that will be supported by specialist housing and tenancy support
  • Brief intervention and short-term support using approaches including the Collaborative Recovery Model
  • Longer term case-managed support that offers a flexible approach to longer-term engagement in order to build trust.
  • Service navigation and referral that focuses on supporting connection with services that will meet an individual’s long-term needs - GPs, clinical mental health services, employment services, legal services and the NDIS
  • Collect, collate and maintain data on consumer contact

What will you bring to the role?

  • Experience in working with people with mental health issues and complex presentations
  • Extensive experience in working with homelessness
  • A clear ability to work within and to develop strong collaborative relationships
  • Extensive experience and the ability to engage consumers and carers in a positive manner
  • The ability to set and maintain clear professional boundaries
  • A commitment to consumers’ rights and to consumers’ active participation in planning their service
  • Excellent written and verbal communication skills
  • Knowledge of the recovery model
  • Computer skills

Mandatory requirements:

  • Police check (refer to PD) – disclosable outcomes considered
  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Fully Insured and Registered vehicle ready for work use (if required)
  • A Disability Worker Exclusion Scheme Check will be undertaken prior to commencement
  • Neami National will require, as part of our condition of employment, that all candidates applying for any position must be fully vaccinated for COVID-19 (or have received one dose of an approved COVID-19 vaccine and have a scheduled appointment to receive their second dose.)
  • Candidates will be required to provide appropriate evidence of COVID-19 vaccination to Neami National.
