Towards Home+ as part of the Victorian Homelessness and Rough Sleeping Action Plan (2018), has the following new suite of services:
As a Community Rehabilitation and Support Worker, you will provide assertive outreach support to people who are homeless and sleeping rough. This includes intensive, flexible, responsive, client-focused support with the aim of assisting clients to successfully sustain housing, improve physical and mental health and develop connections within the local community.
Neami National is a community mental health service supporting people living with mental illness to improve their health, live independently and pursue a life based on their own strengths, values and goals.
