About the Role

  • Full Time, Maximum Term contract until 30 June 2021
  • CSD Level 2 from $61,529 - $66,116 per annum
  • Location: Hurstville

Community Rehabilitation and Support Workers provide a range of rehabilitation and support to consumers, tailored to meet their individual needs. Neami National's work with consumers occurs within their own community of friends, family and neighbourhood. You will work closely with clinical case managers and other community partner organisations in order to deliver the best possible comprehensive service to consumers.

The role will focus on supporting consumers in the Continuity of Support (CoS) program: psychosocial support to people who have previously accessed support from Partners in Recovery (PIR), Personal Helpers and Mentors (PHaMs), and Day to Day Living (D2DL), and have been found ineligible for supports under the National Disability Insurance Scheme (NDIS).

Core duties

  • Engage consumers, using a strengths-based approach to complete a mental health status measurement and a needs assessment.
  • Assist consumers to access targeted individual support and socially based capacity building group activities that focus on social connection, employment, self-management, and physical health.
  • Maintain up to date records and assessment outcomes and risk management processes mandatory to the program, and Neami service delivery model.
  • Engage consumers and develop trusting and professional relationships, with the key aims of
    • Increasing personal capacity, confidence, and independence;
    • Increasing social participation;
    • Streamlining access to appropriate services; and
    • Providing flexible and responsive support at times of increased need.
  • Work within a holistic framework taking into account the needs of consumers, family, carers and other members of the community in order to ensure the service will be flexible to best meet client needs, using a trauma informed approach to encourage safety, trust, choice, collaboration, respect and empowerment.

What will you bring to the role?

  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives.
  • Knowledge of complex mental health and drug and alcohol issues.
  • Knowledge of the NDIS application process
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

Mandatory requirements

  • Minimum Certificate IV in Mental Health or equivalent
  • Current NSW Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
