About the Role

  • Location: Adelaide CBD
  • Maximum Term Contract until 30 June 2022

The Continuity of Support program (CoS) is designed to provide ongoing support for clients who previously received support from Partners in Recovery (PIR), Support for Day to Day Living in the Community (D2DL) and Personal Helpers and Mentors (PHaMS) who are ineligible to receive services under the National Disability Insurance Scheme (NDIS). The COS program will provide psychosocial support for this cohort of clients with severe mental illness who may have an episodic rather than permanent psychosocial disability.

Community Rehabilitation and Support Workers provide a range of rehabilitation and support to consumers, tailored to meet their individual needs. You will work closely with clinical case managers and other community partner organisations to deliver the best possible comprehensive service to consumers.

Core duties

  • Provide direct support and rehabilitation to consumers within their community
  • Working with community partners
  • Maintain records and resources
  • Engage consumers and develop trusting and professional relationships
  • Collect, collate and maintain data on consumer contact

What will you bring to the role?

  • Relevant experience in a similar role
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives
  • Knowledge of complex mental health and drug and alcohol issues
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

Mandatory requirements:

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
  • Mental Health Qualifications (Mental Health (or equivalent Human Services or Community Services) qualification of Certificate IV level or above – more details in the position description
