About the Role:

  • Full Time Maximum Term Contract until 30 June 2020
  • Monday and Friday 9.00am – 5.30pm

The Housing and Accommodation & Support Initiative provides community-based support to people living with a mental illness in Western Sydney as well as providing in-reach and transitional support to people leaving acute hospital admissions as part of the HASI service. Community Rehabilitation and Support Workers provide a range of rehabilitation and support to consumers, tailored to meet their individual needs.

Core duties:

  • Provide direct support to consumers so they gain/maintain a sense of safety and wellbeing
  • Together with the consumer regularly monitor their progress towards their identified needs
  • Work to the principles that underpin the Collaborative Recovery Model (CRM) protocols
  • Engage consumers and develop trusting and professional relationships
  • Work closely with clinical case managers in order to deliver the best possible comprehensive service to consumers
  • Work within a holistic framework taking into account the needs of consumers, family, carers and other members of the community and provide culturally appropriate service responses

What will you bring to the role?

  • Relevant experience in a similar role
  • Experience and working knowledge of homelessness
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives
  • Knowledge of complex mental health and drug and alcohol issues
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

Mandatory requirements:

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
