About the role

As our Support Coordinator, you will work closely with customers to understand their NDIS plan, engage with appropriate service providers and allied health professionals, and monitor progress toward the achievement of their plan goals. An important function of Support Coordination is to assist customers to identify community or mainstream supports that can also assist them to achieve their set plan goals.

Role requirements may include, but not limited to, the following:

  • Work with NDIS customers and their families to assist them to meet their goals and get the best out of their NDIS plan
  • Develop sound plan implementation practices for the duration of the NDIS plan.
  • Build and develop strong relationships with a range of service providers and allied healthcare professionals
  • Develop strong relationships with customers and their families throughout the life of the plan enabling strong levels of choice and control
  • Provide plan review reports to the NDIA clearly articulating the customers’ needs throughout the life of the plan and beyond.
  • Deliver services to customers which are effective, both responsibly and transparently.
  • Assist customers to build their capacity around the understanding of their plan and implementation
  • Work well as a part of internal and external team, communicating customer needs
  • Work with the organisation to follow processes with innovation to develop as required
  • Follow best practice within the organisation ensuring that we maintain integrity around legislation and NDIS guidelines
  • Meet individual performance KPI’s as set by the organisation

About you

The successful applicant will:

  • Possess and demonstrate sound disability industry (or allied health/welfare/education/mental health) experience, knowledge.
  • Demonstrated understanding of the NDIS as well as a clear understanding of the role of a support coordinator
  • Have experience in coordinating the provision services to customers and their families through networking and negotiation with service provides, allied health professionals and any other stakeholders.
  • Have knowledge of the State Disability Services Act and the 2014 Amendment relating to restrictive practices, or the willingness to learn.
  • High-level interpersonal skills and experience supporting people in situations of high emotions.
  • Demonstrate a high level of interpersonal and communicative skills and ability to develop strong relationships with all stakeholders.
  • Demonstrate problem-solving skills to successfully identify problems, develop solutions and implement these using a logical and systematic approach by collaborating with customers, families and stakeholders.
  • Demonstrated ability to work in a fast-paced environment with regular changes in priorities.
  • Demonstrated ability to work as part of a team.
  • Demonstrated experience in the use of Microsoft Office Programs.

Mandatory requirement

Please note that the successful applicant will be required to hold or be willing to obtain a current QLD Driver’s Licence, Current First Aid and CPR, Positive Notice Card from Disability Services (Yellow Card), Working with Children Check (Blue card) and in some circumstances National Criminal History check. You must also have NDIS

Salary sacrifice + Meal entertainment

Attractive Salary packaging including $15,900 packaging limit per FBT year (1 April- 31 March) and $2,550 per year for meals and entertainment expenses.
