About the opportunity
Working with the HealthPathways Melbourne team across North Western Melbourne and Eastern Melbourne PHNs, the Program Officer will work with GP clinical editors and other key stakeholders to develop, implement, monitor and evaluate the HealthPathways program. The program supports the delivery of best practice, evidence-based quality and safe care for patients through:
- mobilising partners across the region to a common goal through collaboration
- developing gold-standard clinical and referral pathways
- working together as a collective team
- living our PHN values
The result is a free website with relevant evidence-based information on the assessment and management of a range of clinical conditions, including local referral guidance that is designed to be used by health professionals at the point of care.
A typical day for the Program Officer may involve:
- Organising local Clinical Working Groups with local leaders, from clinicians to the community, government, professional and peak bodies
- Meeting with GP Clinical Editors and subject matter experts to support the development of referral pathways
- Using project management tools to communicate project progress and problem solve issues that arise
- Liaising with your manager and team, contributing to the continuous improvement of the program
