The Project Officer will support the development and implementation of the Strengthening Care for Children (SC4C) project in 11 general practices. Working with project partners Murdoch Children's Research Institute and the Royal Children's Hospital the SC4C project is a model of care trial in general practice to improve child health outcomes through provision of specialist support. The model aims to increase GP confidence and knowledge in paediatric care and reduce paediatric referrals to hospital and is part of a broader National Health and Medical Research Council research project.

Key responsibilities include:

  • Actively supporting general practices to adopt a new model of care
  • Actively supporting general practices to undertake intensive whole of practice quality improvement activities
  • Working with project partners using a project management approach and ensuring consistent and high-quality project delivery across intervention sites
  • Supporting practices to complete data requirements of the project including use of data extraction tools, data submission and analysis
  • Working with the NWMPHN Workforce Development team to plan, coordinate, facilitate and deliver education to support the project

The successful candidate will have:

  • Qualifications or relevant experience in primary health care or health care
  • Working knowledge of general practice, including business models, funding and income streams, and practice accreditation
  • Project and change management experience, including implementing strategies for effecting change, controlling change and helping people to adapt to change
  • Experience in quality improvement methodology and implementation

PHNs have been established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improving coordination of care to ensure patients receive the right care in the right place, at the right time.

The successful candidate will be required to undertake a National Police Check and a Working with Children Check.
