About the Job

Are you looking to make a positive impact in the lives of older people in the community? Are you interested in developing innovative programs using, music, art and movement for people experiencing memory loss?

We are looking for a hardworking, dynamic individual with exceptional customer service skills. You will have an understanding of Commonwealth Home Support Programs (CHSP) and Individual Support Plan development as well as dementia and memory loss.

Mosman Council Community Care is a small team committed to supporting community members to remain independent in their own home. As part of the team, you will:

  • Be responsible for intake, assessment, and coordinating a range of social support programs.
  • Manage incoming referrals and provide information and ongoing referral.
  • Coordinate a team of volunteers that provide 1:1 assistance and support to community members.
  • Plan events for providers showcasing our services.

What you need to succeed?

As an experienced practitioner you will have:

  • Experience of working with seniors including intake, assessment and care coordination
  • Tertiary qualifications in Social Work, Psychology, Community Services or other relevant qualification
  • Exceptional customer service skills and a ‘can do’ attitude
  • Strong coordination skills and the skills to support a group of volunteers
