Community Rehabilitation - GEM@Home Program

The Community Rehabilitation team offers both home and centred based therapy and sits within the wider Directorate of Community Care. The GEM@Home Program is a new program for Monash Health and involves delivering care to clients in the home as part of a team of nursing, medical, pharmacy and allied health clinicians. The Team Leader will work closely with all team members to ensure that best practice client care is achieved while ensuring the achievement of operational effectiveness and strong partnerships within and outside of Monash Health.

This role is supported by the Manager, Community Rehabilitation.

About You

You are an experienced Allied Health Professional with a passion for assisting clients to live their best lives. You relish the opportunity to work creatively and autonomously with clients in their own homes and enjoy complex problem solving with your inter-professional team. Your are an experienced or emerging leader looking for a new challenge.

What You Need:

  • Bachelor of Physiotherapy or Occupational Therapy, Bachelor of Applied Science (Physiotherapy or Occupational Therapy) or recognised equivalent degree
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Physiotherapy or Occupational Therapy Board of Australia (practicing, no restrictions)
  • Working towards, or completion, of relevant post graduate study
  • Member of the Australian Physiotherapy Association, including involvement in relevant Special Interest Group/s
  • Current Victorian Driver’s Licence
