Monash Health Community supports our community to improve and maintain health independence and wellbeing.

As part of a state-wide initiative our community health nurses work collaboratively with a range of key service providers to provide timely and accessible responses to victim/survivors of sexual assault, family violence and their families. We are now looking to appoint a passionate nurse to join our team, working across two MDCs at Dandenong and Seaford with travel to the youth and family office in Dandenong from time to time. This ongoing role will be appointed on a part time basis, 60 hours per fortnight.

About The Role

Our MDC Community Health nursing service is provided on both an inreach and outreach basis from each MDC and your clients will frequently be those who don't engage with mainstream community health services. In this senior clinical role you will focus on assessment and care coordination to ensure the delivery of best practice health care services to sexual assault survivors.

As part of a small team and frequently working autonomously you will:

  • Identify clients' needs through conducting holistic health assessments
  • Develop and implement client-centred care plans
  • Provide health education
  • Increase health literacy
  • Assist clients to transition into universal health care.

About You

An experienced nurse with a passion for advancing patient care within a vulnerable client base, you have sound knowledge of the health care system and in particular the social model of health. You are an excellent communicator and work collaboratively with your team members and have a broad understanding of the needs of victims of sexual assault, family violence and their families. In addition you are committed to providing integration of care across the care spectrum and can demonstrate a high level of IT literacy.

This role would suit an experienced Community Health or sexual and reproductive health nurse who has worked with a wide range of patients, who may be looking to increase their knowledge and expertise around trauma informed practice. Qualifications and/or experience in primary health, health education, case management, or women's health, mental health risk assessment, family violence and sexual and reproductive health will be advantageous.
