Your Opportunity

This role is to provide intensive case management support to people to sustain new pathways out of homelessness.

Your Key Responsibilities will be to:

• Respond to referrals, obtain informed consent and undertake initial registration paperwork. 
• Actively participate and engage in interagency meetings and forums for Aboriginal people.
• Engage in assertive outreach with people sleeping rough in Sydney’s inner city.
• Engage in weekly patrol with other services in various locations in Sydney where rough sleepers may need support.
• Work collaboratively with Community Liaison team to take referrals and provide ongoing client support.
• Contribute to the effective functioning and development of service through training and development of staff.
• Contribute and complete range of internal and external reports relating to clients and the program including risk assessments.
• Provide case management and coordinated care for each client by creating and updating individual files, making referrals and working collaboratively with other services.

Requirement for Success 

• Degree in Social Work / social science or psychology or minimum 3 years’ of experience in similar role with certificate in AOD; case management.
• Superior case management skill.
• A comprehensive understanding of the issues and needs of service users.
• Strong written and verbal communication skills.
• Demonstrated ability in Microsoft Word, Excel, Outlook.
• Current Driver’s License.
• COVID-19 vaccination proof.
