About the role

We are looking for a Contractor to work predominantly from home and provide phone-based NDIS support coordination services to customers. The role requires some travel for face-to-face support when required.

You will:

• Proactively manage a growing customer base ensuring all agreed support coordination hours are delivered in a way that provides most value to the customer
• Support customer wellbeing by providing personalised support coordination services
• Develop and continually grow a network of local service providers
• Assist customers to define their goals and aspirations identified in NDIS plans.
• Undertake necessary assessments in line with the customers funding, need and choices.
• Provide information, advice, support and mentoring to families and carers to assist them to access services relevant to their health and social needs

What you’ll bring

• Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field
• Registered ABN
• Suitable Public liability insurance
• Work within a recovery oriented framework to support individuals, families and carers
• Develop strength based assessments with clients
• Facilitate the re-engagement and maintenance of family and carer relationships
• Develop daily living skills and capacity for self-care
