Mind offers a targeted range of supports and services that aim to maximise recovery outcomes including group education and learning, mentoring and coaching, education and vocational support, housing and linkage services, counselling and NDIS planning and access supports. Your new role

  • To ensure service delivery to our clients is always our first priority
  • Support customers to identify and achieve their daily living needs and goals
  • Undertake assessments in line with the customers funding, need and choices
  • Collaborate with other services to provide personalised Support Coordination
  • Provide service to customers in line with Mind’s Model of Recovery.
  • Support referrals and service agreements with customers support networks
  • Bring knowledge and experience of the Fee for Service NDIS model

What we are looking for

  • Passionate Mental Health Practitioners
  • Team players – our common goal is our customers!
  • Proven experience in developing strength based assessments
  • The ability to build relationships with customers and their family and carers
  • Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field
