The Community Mental Health Practitioner will deliver high quality, recovery oriented, direct psycho-social support through individual and group work to customers and their families and carers, consistent with Mind organisational values and model of recovery oriented practice.
The Community Mental Health Practitioner is responsible for assisting the Service Manager with the development and implementation of strategies to improve the capacity of general practice and other Community Mental Health Practitioners to deliver appropriate services, increase awareness and understanding of initiatives to ‘close the gap’ in Indigenous health outcomes.
