About the role

The Sub-Acute short stay Adult Step-Up and Step-Down (ASUSD) program is delivered through partnerships with QLD hospital networks. Adults between the ages of 25-65 years are able to access Mind staff 24 hours a day whilst supporting connections with family and friends as well as learning strategies to live fulfilling lives within their community. ASUSD provide a schedule of group activities alongside individual 1:1 support based on strengths and goals identified in recovery plans.

We are seeking an empathetic, driven Community Mental Health Practitioner - Team Leader for a permanent full time, opportunity to work within a close knit team in the Gladstone area. Guiding and managing a dedicated, close knit team you will act as the practice lead to ensure delivery of holistic recovery oriented, person-centred services to complex care clients.

Key responsibilities

  • Mentor and lead effective practice to staff working with clients, provide support to families and carers, and plan and deliver group work.
  • Coach the team to deliver high quality services and facilitate positive educational and social experiences.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Manage administrative tasks including rostering, leave coverage, facilitating handovers and planning budget expenditure.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
  • Capability to effectively lead and develop a team to achieve best quality service outcomes and provide professional supervision.
  • Demonstrated understanding of the Community Services sector and capacity to build strong relationships with service providers, clinical teams and key stakeholders.
