About the role

Service delivery to our clients is our first priority! As a Mental Health Practitioner, you will be responsible for providing recovery focused mental health services, customer wellbeing and ensure our clients and their families have the support and services they need to achieve their goals. Knowledge of the Fee for Service NDIS or TAC models are essential.

You will:

  • Bringing your knowledge and experience of the Fee for Service (NDIS) or TAC models
  • Support customer wellbeing by providing personalised support coordination services or other Fee for Service support
  • Assist customers to define their goals and aspirations identified in NDIS plans
  • Undertake necessary assessments in line with the customers funding, need and choices
  • Provide information, advice, support and mentoring to families and carers to assist them to access services relevant to their health and social needs

Skills & experience

  • Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field
  • Work within a recovery oriented framework to support individuals, families and carers
  • Develop strength based assessments with clients
  • Facilitate the re-engagement and maintenance of family and carer relationships
  • Develop daily living skills and capacity for self-care
