About the role

A permanent part-time position has become available for an innovative and driven practitioner to support clients in a residential setting for Mind Australia in partnership with clinical partner Monash Health.

Prevention and Recovery Care (PARC) is a modern and purpose built service provided in a home-like environment, offering a combination of recovery focused specialist community and clinical mental health services, using the expertise of both Mind and Monash Health.

Reporting to the Service Manager, you will provide comprehensive assessment of client needs, individual recovery plan development and review, delivery of individualised client support, short-term self-management and carer support. This position works across a 24 hour roster, 7 days a week. You must be available and willing to work across all shifts.

Duties

  • Working with clients to enable them return to their place of residence and a meaningful life through supporting them through a staged approach to recovery
  • Work within a recovery oriented framework to support individuals, families and carers
  • Working collaboratively with a broad range of services as they relate to client’s needs to ensure a co-ordinated and integrated response to the client’s recovery goals
  • Work in partnership with clinical services and other key stakeholders
  • Planning and developing group work programs that will assist customers to build their skills, focus on their recovery

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field as designated by Mind
  • Demonstrated ability to build solid connections with clients, colleagues and Clinical Partners to implement a recovery oriented framework
  • Previous experience working with clients experiencing mental illness or in a similar role
  • Experience overseeing the facilitation and design of group work aligned with individual recovery plans
  • Strong verbal and written communication skills
