About the role

The WA Mental Health Commission in conjunction with Health Service Providers (HSPs), Non-Government Organizations and other stakeholders within the mental health and AOD sector have developed a pilot community-based model - Active Recovery Team (ART). ART is an active recovery approach delivering care by an integrated team providing intensive, highly coordinated flexible support and treatment to individuals. The program provides a responsive, tailored support over a 90-days that involves multidisciplinary teams bringing together clinical mental health services along with community-based organisations to provide integrated care options outside of a hospital setting.

We are seeking a Community Mental Health - Lead Practitioner for a 12 month fixed term, full time opportunity for this new program in the Rockingham area. You will provide practice leadership and direction to Mind staff in the delivery of the psychosocial rehabilitation, recovery programs and services as well as ensure staff are supported in their work. The Lead Practitioner will be responsible for delivery of person-centred services to clients, families and carers in line with Mind’s model of Recovery Oriented Practice.

Key Responsibilities

  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Assist clients to return to a place of residence and a meaningful life through a staged approach to recovery.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Contribute to program planning and ensure planned clinical appointments and reviews are coordinated and facilitated.
  • Collaborate with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Experience working directly with people with mental health issues, complex needs and with their families and carers.
  • Demonstrated capacity to effectively lead and develop a team to achieve quality service outcomes including experience in the provision of supervision.
  • Demonstrated ability to plan and prioritise to meet consumer service delivery requirements.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
