About the role

Headspace in partnership with Mind aims to promote and facilitate improvements in the mental health, social wellbeing and economic participation of young people aged 12 - 25 years residing in the North Eastern suburbs. Services in this youth friendly space are delivered across four core streams, primary health care, mental health, alcohol and other drug (AOD), and social/vocational, to improve access for young people to mental health programs as well as ensuring better co-ordination between service providers.

We have a permanent, full time opportunity for an experienced, motivated Community Mental Health Practitioner – Lead Practitioner Intake & Assessment to join the passionate Headspace team located in Greensborough and Plenty Valley. The Lead Practitioner will act as a first contact point for young people accessing the Centre and build positive, supportive relationships with the clients and their family and friends.

Key Responsibilities

  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Operate collaboratively with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
  • Effectively engage, screen and assess, organise referrals, provide brief interventions and coordinate care for young people that contact or are referred to the Headspace Centre.
  • Provide outreach to schools and agencies as well as consultations with young people, their families, and professionals.

What you’ll bring

  • Tertiary qualifications in Allied Health (Occupational Therapy, Social Worker, Psychology or Psychiatric Nursing) or other health related field and eligibility to obtain AHPRA registration.
  • Proven experience working with young people and or adults and families with mental health and/or contextual challenges (family violence, poverty, homelessness, drug and alcohol, protective issues).
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
