The Community Mental Health Practitioner will work with young people to define their own outcomes whilst providing individual and shared support within a residential setting. The role supports customers to achieve their recovery goals through one-to-one support, facilitating learning workshops and assistance with daily tasks in a shared living arrangement. The role also plays an integral role in developing individual programs; plans that outline goals, aspirations and desires working towards recovery and independent living.
