About the role

The Rockingham subacute service is a ten bed residential service located in the community of Shoalwater. The service is delivered in partnership with Southern Metropolitan Health Service and Access Housing. The Rockingham service offers a short term (up to 28days) recovery focused residential service for people who are either leaving acute mental health care, or who would benefit from 24 hour support to avoid a hospital admission.

The Community Mental Health Practitioner – Family Engagement Worker ensures that the families and carers of clients of the services are welcomed, included and involved in the support provided to their loved one. The role works closely with clients, team members, and Clinical Partners to provide information and support to families, and where required provide meaningful referral to external providers.

This is a permanent part time role working roughly 21 hours per week. The successful applicant will be required to work Monday-Thursday from approximately 1pm-6pm.

Key Responsibilities

• Providing services to clients, families and carers in line with Mind’s Model of Recovery Oriented Practice and Family Inclusive Framework.
• Coaching and mentoring the team to ensure a shared understanding of the client’s individual recovery plan.
• Building rapport and developing trusting relationships with clients, families and their carers.
• Working in partnership with clinical services and other key stakeholders to engage with families and carers of clients.
• Making linkages and support relationships including referral pathways, that support access & connectedness for PARC clients, their families and carers, and the community.

What you'll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field. Bachelors degree is preferred.
• Proven experience of successfully working with families in a recovery orientated framework.
• Demonstrated in-depth knowledge and understanding of the principles and challenges of supporting vulnerable families.
• Strong verbal and written communication skills including the use of Microsoft/other software.
• Demonstrated understanding of the Community Services sector and ability to build strong relationships with clients, agencies, clinical providers and key stakeholders.
• Skills and experience in advocacy, collaboration, facilitation, evaluation and problem solving.
