About the role

The Yandina Transitional Housing and Mental Health Accommodation Pathways at Discharge (MHAPD) outreach services sit within Mind’s Sub-Acute program and operates as a client centred framework providing services to clients with severe mental illness. These services provide recovery based interventions aimed at successfully supporting clients to address and overcome barriers that contribute to homelessness or increased risk of homelessness.

We are seeking a compassionate, self-motivated Community Mental Health Practitioner for a permanent, full time opportunity to work alongside the empathic MHAPD team in the Box Hill and Eastern Suburbs area. Working from home, you will be integral in assisting clients in the community to access accommodation services and support mental health recovery.

Key responsibilities

• Provide direct out-reach support to individual clients through recovery based interventions.
• Support clients by delivering high quality service and facilitating positive educational and social experiences.
• Operate collaboratively with a range of local services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
• Work as part of a multidisciplinary team offering specialised psychosocial support.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
• Expertise in working with people with mental health issues, issues relating to homelessness, complex needs and interacting with families and carers.
• Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
• Relevant experience in facilitating group activities for clients experiencing mental ill health.
