About this opportunity

Newly created, the Service Navigator role exists to improve accessibility to the health system for people with severe mental illness and associated psychosocial disorders.

Building and drawing upon Wellways' Community Recovery Model, the Service Navigator will establish and enhance coordinated referral processes to support consumer access to psychosocial, clinical and primary healthcare. This will include the development and distribution of up-to-date and detailed information resources to assist consumers to access General Practitioners (GPs), Aboriginal Community Controlled Health Services (ACCHS), and well as community support services focused on psychosocial support needs such as housing, employment, education, family support and financial matters. The Service Navigator will work closely with NDIS Local Area Coordinators and the Murrumbidgee Local Health District to gather and disseminate information on local services and identify service gaps.

The Service Navigator will achieve the following outcomes:

  • Consumers, families and carers will have a better understanding of the service options available across a range of service domains
  • Consumers will have an increased choice in accessing a broader range of relevant health and support services that enable them to achieve recovery goals and manage their conditions on a day to day basis in the community
  • GPs, program managers and service provider staff will have a better understanding of the services available in their area that support the social, mental and physical health needs of consumers with severe mental illness and promote effective multidisciplinary care
  • Wellways, MPHN, MLHD and NDIS Local Area Coordinators will develop a joint understanding of psychosocial consumer referral pathways, available supports, service gaps and emerging issues
  • Where there is capacity, Wellways, MPHN, MLHD and NDIS Local Area Coordinators will implement strategies to mitigate identified barriers

About you

You will need relevant qualifications or commensurate experience in working in a support role for people with a mental illness, generally a Certificate IV qualification or above. You will need a current and valid Driver's License and are able to undertake travel. You will also need a satisfactory Police Records Check and Working with Children Check as well as the NDIS Worker Screening Check.

Desirable but not essential: Previous experience working within Aboriginal/Culturally and Linguistically Diverse (CALD) communities and/or understanding of Aboriginal/CALD communities, both socially and culturally. Experience working with older people will also be looked on favourably.
