Your new role

The Team Leader - Prevention and Chronic Illness Care Team will be responsible for:

  • Providing leadership and direction to the multidisciplinary staff in day to day operations of the team.
  • Support the team to deliver high quality, evidence based services to the community and in line with Merri Healths strategic directions.
  • Support the implementation of innovative, appropriate, effective and efficient models of care.

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • Tertiary qualifications in Allied Health or Nursing
  • Post graduate qualifications in a relevant field or equivalent experience
  • Demonstrated ability in leading a multidisciplinary team
  • Demonstrated understanding of chronic disease frameworks and the use of best practice guidelines around prevention, risk factor modification and chronic disease management.

What we offer

You will be leading an established multidisciplinary team who provide high quality and evidence based services to clients who are aged under 65 (under 50 for Aboriginal and Torres Strait Islander People) including children. Of key attention will be working with the Manager and staff to develop pathways and improvements that support effective Program structure, Prioritisation of need, Program delivery, Partnership development and Program monitoring. The Team Leader Prevention and Chronic Illness Care Team will also work closely with the other Prevention and Complex Care Team Leader’s to achieve key program objectives.
