Your new role

You will be working as part of an established multi-disciplinary team to provide individual counselling in smoking cessation clinic which targets people who wish to stop smoking, especially those with chronic diseases. This role is funded by Community Health and will require using an integrated addiction counselling model.

As the Smoking Cessation Facilitator in the Prevention of Chronic Illness Care Team you will be responsible for:

  • Providing high quality, evidence-based individual counselling service to assist clients in the cessation of tobacco smoking
  • Develop goal direct care plans to assist clients in achieving “their goals” and promote a self-management approach
  • Health education, on both a group and an individual basis, with a particular focus on chronic conditions and restoration of function
  • Participation in planning, designing and implementation of group programs that operate across different sites of Merri Health

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • Qualification in relevant Allied Health, Social Work or Counselling field and current registration with AHPRA or appropriate registration body
  • Demonstrated experience providing smoking prevention and cessation practices
  • Ability and experience to develop, plan, implement and evaluate smoking cessation programs
  • Experience or training in motivational interviewing or health coaching models
  • Demonstrated ability to work autonomously and as part of a multi-disciplinary team
