Your new role

As a HARP Care Coordinator you will be responsible for:

  • Conducting comprehensive client assessments to identify health and psycho-social issues and risks for clients with chronic conditions, complex co-morbidities associated with advanced age and a range of complex psycho-social issues.
  • Providing client-centred care planning to identify barriers to self-management and independent living.
  • Make referrals to services to address these issues and coordinating the client’s care as part of a multidisciplinary team.
  • Providing services in clients’ homes, community health services and outpatient settings.

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • A commitment to Merri Health values and philosophy, and ability to engage in the enhancement of Merri Health culture in meeting organisational objectives.
  • Tertiary qualifications in nursing, social work, physiotherapy or occupational therapy.
  • Experience in generic care needs assessment, care planning and care coordination for people with complex and chronic conditions across the acute-community continuum.
  • Experience is providing outreach, home-based services.
  • Ability to be flexible and work well in a multidisciplinary team as well as autonomously.
