About the Role

The Senior Outreach Case Worker works with a multidisciplinary team to provide therapeutic support, assessment, psychological interventions, case management and practical support to young people 12-25 years and their families.

You will be required to work collaboratively with the Youth Mental Health Outreach (YMHO) team and assist the YMHO Clinical Lead with administrative and supervision duties to ensure efficient provision of services. The role will support this new youth mental health service implementation, and ongoing codesign. This role is being offered on a fixed term full time contract.

Minimum Requirements

  • Tertiary Degree in relevant Human Services field (i.e. Psychology, Social work, Registered Nurse in Mental Health, Occupational Therapy or a related discipline)
  • Current registration with an approved professional body
  • Previous triage experience is highly desirable
  • Demonstrated significant case management experience working in a mental health setting with complex young people and their families
  • Experience and knowledge in the areas of financial support, education, substance abuse, working with other cultures, family work and homelessness
  • Experience and knowledge working from a trauma informed practice approach
  • Working with Children Check
  • Driver’s Licence
  • National Police Clearance (obtained within the last six months)
